Kid's Fun Zone Birthday Party Request Form
We look forward to hosting your child's next Birthday Party at the Kid's Fun Zone.  Please fill in the details for your child's Birthday
Party and then click SUBMIT at the bottom of the page.  We will have a Birthday Party Specialist call you once we have
researched our available dates and times based on your requests listed below.
Birthday Parties Terms:
$50.00 Deposit is required to hold your date and time slot.
This is Due 1 week prior with final payment day of party.
Cash, Visa, Master Card only. No personal checks are accepted.
Minimum of 8 Guests.
Parental supervision is required at all times.
Please arrive 15 minutes early.
No outside food or drinks are allowed in the facility.
We can provide an Ice Cream Cakes or you can bring your own.
Final party count is required 48 hours prior to the party. Smoking is not permitted in the Fun Zone.
We are not responsible for lost or stolen articles.

Parties are NEVER confirmed until payment is processed so DO NOT send out invitations until we confirm your payment is
